feat(frontend): 孵化梯接真——删 ladder/todos mock+毕业口令表单+闸门阈值页+评审按钮 [nas] [no-doc]

spec §4.5 决议 L 六件之 4 前端侧:适配层签名不变;GateConfig 两级阈值+审计;
graduate 走后端口令闸门;三按钮留痕 registry_events。
